How do I print a doctor’s physical form?

Who can help me with sports related questions regarding my student and the school they attend?

See the HOME tab to find the Site Athletic Directors email address of the school your student attends. You can then email the site AD or talk with your students coach.

What forms need to be filled out for my student to start a sport?

See the “ ELECTRONIC PARTICIPATION FORM ” tab for the form to fill out on-line.

What forms do I need to turn in to my coach or school Athletic Director?

The only form (hard copy) that needs to be turned in is the Physical Form signed by the doctor. NEW……HB 2016 form has been added and must be filed out. You can find it under the “ ELECTRONIC PARTICIPATION FORM ” tab.

NOTE: It may take 24-48 hours for a newly enrolled student to appear on the RankOne system.
